The Online Accommodation Request Form is closed and is undergoing changes in preparation for the 2010-2011 school year. The Online Accommodation Request Form will be available soon. In addition, a training document will be updated to explain any new features of the system. A broadcast email will be sent to District Testing Coordinators announcing the opening of the system.
A training site is available to familiarize yourself with the online system prior to completing an accommodation request. No accommodation request will actually be submitted. The Accommodations Request form will require javascript in order: to provide feedback to your selections; to perform data entry validations; and to insure proper attachment of files.
