The accommodation request process utilizes e-mail to submit, confirm, and approve/deny accommodation requests.

Once a determination has been made, the decision is valid during an entire testing cycle, including all retests. Do not submit an accommodation request for a retest UNLESS it is a new request or there has been a change to the student's accommodation needs.

Do not include confidential student information (e.g., first and last names, PEIMS numbers, social security numbers) in your request as this is a violation of the Family Educational Rights and Privacy Act of 1974 (FERPA).

The Online Accommodation Request Form is best viewed with a screen resolution of
1024 by 768.


HOW TO: To enter compatibility mode with Internet Explorer version 8 (IE8), use one of the following methods:

  1. Click the IE8 Toolbar compatibility button located near the Refresh and Stop buttons.
